Ezine ID: * Sumbit one or more ezine IDs (separated by space).
Ask for Name (If checked, the Name field will be included in the form.)
Ask for Subscription Format (Plain Text or HTML) (If checked, a dropdown menu will be added for Plain Text or HTML subscription format selection.)
Ask for Action (subscribe or unsubscribe) (If checked, a radio button will be added to select action - join or leave the ezine.)
Include Ezine Title (If checked, the ezine's title will be included; this option is always "checked" for multi-forms).
